Position Summary:
The Family/Internal Medicine Clinic LPN provides comprehensive patient-centered nursing support within Trinity Healths primary care setting under the direction of providers RNs and the Clinical Services Manager. This role assists with patient intake clinical procedures point-of-care testing and care coordination to support efficient well-organized patient visits. The Family/Internal Medicine Clinic LPN works collaboratively with the care team to promote continuity of care through clear communication accurate documentation and timely follow-up with patients. Through reliable clinical practice teamwork and adherence to established standards this position contributes to high-quality primary care services and positive patient outcomes.
Key Responsibilities:
- Perform patient intake procedures including vital signs health histories documentation updates and preparation for provider examinations.
- Administer medications immunizations and treatments within scope and according to clinic protocols and patient safety guidelines.
- Complete point of care testing collect and package laboratory specimens and ensure accurate documentation in the electronic health record.
- Assist with minor procedures wound care and diagnostic preparation while maintaining a clean and organized clinical environment.
- Support care coordination by reinforcing provider instructions scheduling follow up needs and communicating changes in patient condition to the care team.
- Maintain inventory restock supplies and ensure exam rooms and equipment are prepared to support efficient primary care operations.
Licenses and Certifications Required:
- Current or temporary LPN license in ND or compact LPN licensure
- CPR required within orientation period
- Ongoing Point of Care certifications as required for department required within orientation period
Educational Requirements:
- LPN as evidenced by licensure
Experience Requirements:
- One year or more of LPN experience preferred
- Clinic experience preferred
- Coding experience preferred
Special Skills or Training Requirements:
- May require travel to satellites
- Computer use
- Trinity community clinics - draw and package lab tests
Physical Requirements:
The LPN frequently stands walks bends and moves between exam rooms to support patient flow throughout the clinic. The role requires the ability to lift and carry light to moderate medical supplies and assist with patient positioning or mobility as needed. Adequate vision hearing manual dexterity and clear communication abilities are required to safely perform clinical tasks and interact effectively with patients and staff.
Environmental Requirements:
Work is performed in an outpatient primary care clinic with routine exposure to patients medical equipment and occasional contact with blood or body fluids. The LPN adheres to standard precautions PPE use and infection prevention measures while working in exam rooms and shared clinical spaces. The role may involve variable patient volumes competing priorities and potential travel to satellite locations based on operational needs.
