Summary
The Licensed Clinical Social Worker provides direct patient care in outpatient and nursing home settings collaborating with a multidisciplinary team to deliver compassionate patient-centered evidence-based behavioral health services. This hybrid role requires 2 3 days per month in-office.
Key Responsibilities
- Conduct patient assessments using evidence-based tools
- Provide treatment for behavioral health conditions (CBT Motivational Interviewing crisis intervention Interpersonal Psychotherapy behavioral activation)
- Make treatment recommendations during collaborative care meetings
- Monitor treatment through care planning and coordination with clinical teams and health homes
- Deliver crisis intervention and risk assessments as needed
- Engage clients with limited behavioral health utilization through outreach
- Document services in a timely manner
- Provide clinical supervision or consultation in the absence of the Clinical Manager
- Participate in team meetings case conferences and semi-monthly supervision
- Offer field instruction to MSW students when appropriate
Qualifications
- LCSW licensure
- Master of Social Work degree from an accredited institution
- Proficiency in behavioral health screening tools and biopsychosocial assessments
- Ability to document services in electronic health records and review relevant health data
- Strong communication skills for integrated care teams (PCP NP/MD Health Home Care Management staff)
- Bilingual English/Spanish preferred
