DescriptionJob Title : Care Plan Coordinator - LPN Job Summary :
- The Care Plan Coordinator LPN provide comprehensive assessment/care planning of all residents according to state guidelines input of CP/MDS into EMR System conduction of weekly staffing/care plan meetings and monitoring of MDS completion responsibilities of other departments.
Experience Education Training Special Skills and Licensure :
- 3 years clinical experience as a Licenses Practical Nurse
- Diploma or Associates Degree accredited LPN program
- CPR ;Licensed Practical Nurses
Job Function :
- Program Management and Daily Operations
- Perform thorough and comprehensive assessment of residents within 7 days of admission including physical exam observation input from staff nurse CNA and other departments as needed.
- Completion of MDS sections C D G H I J L M O P Q R within 14 days of admission. Generation of accurate comprehensive Care Plan within 21 days of admission.
- Input of MDS/care plan into EMR system.
- Completion of quarterly reviews for each resident as required by state guidelines. Completion of MDS/Care Plan with significant change of resident as required by state guidelines.
- Lead weekly task oriented staffing meeting involving all disciplines involved in MDS completion.
- Report to DON/ADON any omissions/trends with MDS/Care Plan process involving other departments. Collaboration with ADON for RN signatures weekly on MDS.
- Completion of ancillary forms as needed (restraint use psychoactive med use self admin. of med etc.).
- Quality
- Maintains appropriate records and provides feedback validation and quality reports. Conducts edit checks weekly making any necessary changes while providing follow-up to administration and staff.
- Assists the Director Nurses in maintaining compliance with all DHH/Federal Regulations.
- Other Duties As Assigned
- Performs other duties as assigned or requested.
