JOB SUMMARY

Siloam Missions Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization. The MTCU provides time-limited transitional housing integrated with essential medical and social supports to improve health and housing outcomes.

Guided by person-centered strengths-based trauma-informed practice and rooted in harm reduction and Housing First principles the MTCU operates through a holistic and multidisciplinary approach.

The MTCU Case Worker provides case management medical system navigation and housing support to residents in the unit. Working collaboratively with the Intake and Transition worker residents clinical staff and community partners the Case Worker supports individuals to navigate health care and social systems and successful transition to permanent housing.

KEY DUTIES & RESPONSIBILITIES

Case Management Care Coordination & Advocacy

  • Engage residents using a trauma-informed strengths-based approach to complete assessments and gather relevant history
  • Collaborate with residents to develop and implement individualized service plans addressing health needs goals and housing outcomes
  • Provide ongoing case management to monitor progress reassess needs and adapt service plans as required
  • Support access to services including medical care identification income assistance harm reduction housing transportation and long-term supports
  • Advocate for resident needs within health housing and social service systems
  • Participate in interdisciplinary case conferences and care planning meetings
  • Share resident information explicitly in accordance with consent privacy legislation and organizational policy
  • Work within defined scope of practice while collaborating closely with nursing medical and mental health professionals
  • Connect residents to culturally appropriate services and supports

Medical System Navigation

  • Support residents in understanding diagnoses treatment plans and follow-up care in collaboration with clinical staff
  • Coordinate medical appointments including scheduling reminders transportation and accompaniment when appropriate
  • Act as a liaison between residents and medical providers to support continuity of care and consent-based information sharing
Harm Reduction and Mental Health Support

  • Provide overdose/poisoning prevention education and respond to poisonings according to organizational protocols (including naloxone administration once trained)
  • Support resident-defined harm reduction goals aligned with readiness for change
  • Maintain a non-punitive low-barrier approach to substance use
  • Identify and respond to mental health crises in collaboration with clinical staff and external crisis services
  • Support safety planning crisis response and de-escalation using trauma-informed approaches
  • Recognize personal limits and seek supervisory or clinical support as needed
  • Adhere to all organizational policies related to safety incident reporting confidentiality and ethical conduct
Housing Support

  • Prioritize locating securing and maintaining permanent housing in alignment with Housing First fidelity standards
  • Provide strength-based case management to support long-term housing stability
  • Assist with housing-related moves as required
  • Build and maintain relationships with landlords and housing providers advocating for residents as needed
Administrative & Team Responsibilities
  • Complete timely accurate and objective documentation to support continuity of care risk management and funding requirements
  • Maintain electronic and paper records in accordance with departmental procedures
  • Attend and participate in team departmental and organizational meetings and events
  • Participate in professional development activities
  • Support and mentor student interns as required
  • Perform other related duties related to the role as assigned
EDUCATION & EXPERIENCE

  • Completion of a diploma/degree in a health or human service field such as social work psychology; a combination of education and experience may be considered
  • Certifications in NVCI (Nonviolent Crisis Intervention) ASIST (Applied Suicide Intervention Skills Training) CPR/First Aid Mental Health First Aid (MHFA) and Motivational Interviewing preferred
  • Experience and demonstrated knowledge in homelessness addiction and mental health fields
  • Experience in case management completing assessments service planning and goal setting
  • Experience working within a Housing First framework and an understanding of a variety of housing models including Housing First Recovery-based; transitional low barrier supportive etc.
  • Must have a valid Class 5F drivers license an acceptable driver abstract and a vehicle in good condition
  • Lived experience is an asset
KNOWLEDGE/SKILLS/BEHAVIORS

  • Strong knowledge of community resources related to homelessness poverty health mental health and substance use
  • Advanced understanding of housing resources and systems
  • Demonstrated commitment to harm reduction trauma-informed anti-oppressive anti-racist and culturally safe practices
  • Understanding of Indigenous history colonization and its ongoing impacts on Indigenous peoples
  • Exceptional interpersonal verbal and written communication skills
  • Outstanding relationship-building skills and strong active listening abilities to effectively understand and respond to resident needs
  • Strong de-escalation mediation critical thinking and problem-solving abilities
  • Ability to work effectively in complex high-change environments with patience and resilience
  • Outstanding attention to detail organizational and time management skills
  • Skilled at prioritizing and multitasking
  • Exceptional documentation skills
  • High level of professional judgement integrity confidentiality and accountability
  • Highly developed understanding of and proven ability to communicate appropriate professional boundaries
  • Ability to collaborate and coordinate effectively with multiple departments and functions to support client-centered outcomes
  • Proficiency in Microsoft Office (Word Excel Outlook)
RELATIONSHIP

  • Building professional and respectful relationships with residents
  • Collaborate with other internal departments in the assessment and support of residents
  • Working closely with medical system personnel including emergency room hospital social work primary care providers in community
  • Liaise advocate and collaborate with other agencies and housing providers throughout the sector
WORKING CONDITION

  • Hours of work: 40 hours per week - Monday to Friday; 8 a.m. - 4:30 p.m.
  • A work-issued cell phone will be provided. No on-call responsibilities are required
  • Will be required to drive residents to appointments in your personal vehicle
  • May be required to lift up to 50 lbs. perform repetitive motions (bending twisting lifting etc.) and assist residents with moving related to housing
  • Will require working alone with residents and visiting residents in their homes
  • Work will involve exposure to substance use (including residual smoke) mental health crises medical needs and emotionally demanding situations
  • Hazardous conditions could potentially include unpredictable or risky behaviours from community members
WORKPLACE SAFETY AND HEALTH

The incumbent contributes to making the organization safe for clients and staff and recognizes the importance of reporting unsafe situations and participating in follow-up reviews as a learning opportunity.
  • Provides a safe environment by ensuring adherence to Workplace Safety and Health Regulations and Policies Infection Control Guidelines WHMIS and Safe Work Procedures
  • Immediately investigates and recommends corrective action on any unsafe acts work conditions incidents near misses injuries or illnesses
  • Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facilitys Fire Disaster and Evacuation Plan
  • Supports an environment which avoids prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of services. Community Member safety is a standing item for all individual and departmental meetings
