Physical Therapist (PT) SNF 13-Week Contract Hertford NC

We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting in Hertford North Carolina. The ideal candidate must be flexible and willing to float between Hertford and Windsor NC locations.

Position Details:

Job Title: Physical Therapist (PT) SNF
Location: Hertford & Windsor NC
Contract Length: 13 Weeks
Pay Rate: $45 $50/hour

Requirements:

Active North Carolina PT License
BLS Certification Required
SNF experience Required
Ability to float between assigned facilities

Responsibilities:

Evaluate and treat residents in a skilled nursing setting
Develop and implement individualized treatment plans
Document patient progress and treatment outcomes
Collaborate with interdisciplinary care teams
Ensure compliance with facility and regulatory standards
