Dominican offers emergency services and is a Certified Stroke Center and Chest Pain Center. Dominicans services include the only comprehensive Cancer Center in Santa Cruz County a Total Joint Replacement program and advanced neurological and endoscopic services. Dominican regularly receives A grades for hospital safety from Leapfrog Group and has received national recognition for superior patient safety cardiac care and stroke treatment from Healthgrades a leading provider of comprehensive information about physicians and hospitals.
As Santa Cruz Countys heart attack (STEMI) receiving center Dominican Hospital provides cutting edge cardiac care. The hospitals heart attack treatment times consistently beat the national average and the hospital offers two cardiac catheterization labs available 24 hours a day 7 days a week. Groundbreaking cardiac procedures at Dominican include the Transcatheter Aortic Valve Replacement (TAVR) procedure which allows for heart valve replacement without opening a patients chest.
Dominican Hospital has forged significant partnerships with area hospitals to bring exceptional care to Santa Cruz County. The hospital offers a level III Neonatal Intensive Care Unit (NICU) through a partnership with Lucile Packard Childrens Hospital Stanford. Neurosurgery at Dominican is also offered through a partnership with Stanford Health Care.

- Clinical Practice & Case Management- Provides safe therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care. Integrates knowledge skills and experiences to meet the needs of patients and families throughout the continuum to include patient and family education.
- Responsible for developing overseeing and implementing comprehensive case management and treatment plans for patients with cardiac and pulmonary diseases.
- Utilizes the AIDET (Standard Patient Satisfaction Criteria) to communicate with others with a focus on patients and their families.
- Provides monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with the diagnosis of heart disease COPD diabetes PAD obesity etc.).
- Maintains current clinical data insurance information and program records on each client.
- Adjusts treatment plan in accordance with established protocols co-morbidities client goals/needs and medical history.
Job Requirements
- Bachelors Of Science Nursing and One year experience in lifestyle management exercise training prescription and supervision of medical patients. upon hire and
- One year experience providing nurse case management leading group exercise and education classes and
- Registered Nurse: CA upon hire and
- Basic Life Support - CPR upon hire and
- Advanced Cardiac Life Support upon hire and
Preferred
- Masters Of Science or advanced degree in nursing or related field and Two years experience in lifestyle management exercise training prescription and supervision of medical patients. upon hire and
- Two years experience providing nurse case management leading group exercise and education classes.
