Job Summary
Elevate Healthcare Services is seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health Care team in Virginia Beach this role you will provide high-quality nursing care to patients in their homes helping them maintain their health and independence in a comfortable and familiar environment. This position offers a competitive compensation of $44.00 - $46.00 per hour and the opportunity to make a meaningful impact on the lives of our patients and their families.
Responsibilities
- Deliver comprehensive nursing care to patients in their homes including assessment treatment and education.
- Develop and implement personalized care plans based on patient needs and physician instructions.
- Monitor patient progress and report any changes in condition to the healthcare team.
- Administer medications injections and other treatments as prescribed.
- Provide education and support to patients and their families regarding disease management health promotion and prevention.
- Maintain accurate and timely documentation of patient care and communication.
- Collaborate with interdisciplinary team members to optimize patient outcomes.
Requirements
- Current Registered Nurse (RN) license in the state of Virginia.
- Minimum of 1 year nursing experience preferably in home health care or a related setting.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and manage time effectively in a home setting.
- Excellent communication and interpersonal skills.
- Valid drivers license and reliable transportation.
- CPR certification required.
