Job Summary
Empath Home Health a member of Empath Health is seeking a Home Health Intake Coordinator. The Home Health Intake Coordinator is responsible for efficiently coordinating the referral process ensuring a seamless review response andcarecoordination for patients referred to the Organization.
Location: Work on-site at a Empath Home Health Florida branch.
What Youll Do
- Serves as the primary point of contact for both internal and external customers providing exceptional customer service.
- Receives and meticulously documents patient referrals for home health services ensuring strict adherence to applicable coverage criteria.
- Reviews referrals to determine the appropriate care setting and communicate effectively with the care center Business Development and referral sources as needed.
- Gathers essential information to assist the clinical team in scheduling evaluation visits.
- Communicates pertinent patient information to the care center team through approved processes.
- Coordinates with other departments for insurance verification and obtains payer authorization as necessary.
- Professionally communicates and follows up with referral sources regarding required documentation for a comprehensive referral.
- Meets productivity expectations with quality documentation of complete referrals and all required follow-ups.
- Coordinates the evaluation/admission process and may assign field staff to conduct initial assessments.
- Ensures compliance with policies procedures and regulatory mandates including adherence to the terms of the Organizations Compliance Program.
- Performs other duties as assigned

What Youll Need
- High School diploma or GED equivalent.
- Two (2) years proven experience in Home Health referral management or a related field.
- Clear understanding of medical terminology and basic ICD-10 coding.
- One (1) year of supervisory experience or demonstrated ability to manage a team.
- Proficient in Microsoft suite of products including Outlook Word and Excel.
- Reliable internet service and the ability to work from home with minimal disruption to the workday.
