Job Summary
Independent Living Specialist Olmstead
ARISE/Exceptional Family Resources is looking for a Independent Living Specialist to provide support to individuals who wish to move from nursing home residence back to living in the community. The program provides support services and rental assistance to eligible individuals in Onondaga and surrounding counties. The Independent Living Specialist identifies goals and creates a plan with each program participants to coordinate community-based services and provide meaningful one-on-one support. This position works closely with the programs Housing Specialists to maintain stable housing in a persons chosen community setting.
Required Qualifications:
- Associates degree required bachelors degree preferred or equivalent combination of education and related experience
- Excellent communication and organizational skills
- Experience working with individuals with disabilities required
- Solid knowledge of available community-based services
- Proficiency in Word and Excel general computer literacy to navigate databases use email and other required electronic systems
- Valid NYS Drivers License
Workdays and Times: M-F 8a-4p
Location: Syracuse
Hourly rate: $20.16-$21.16/hour
