Our client is seeking a full-time Home Health Physical Therapist (PT) to join their growing Waco branch. This is a chance to move away from the high-volume conveyor belt feel of a traditional clinic and focus on delivering high-impact one-on-one care where it matters most: in the patients home. You will be responsible for helping patients recover from surgery injury or chronic illness while enjoying the professional autonomy that only home health provides.
Key Responsibilities
- Evaluations & Plans of Care: Perform initial physical therapy evaluations and develop individualized goal-oriented treatment plans.
- Functional Rehabilitation: Implement therapeutic exercises gait training and balance retraining focused on real-world home safety.
- Interdisciplinary Collaboration: Work closely with RNs OTs and Physicians to ensure a comprehensive approach to patient wellness.
- Patient & Family Education: Train caregivers and patients on home exercise programs (HEP) and the safe use of assistive devices.
- Documentation: Utilize mobile EMR technology to document visits and progress in real-time ensuring compliance with CMS standards.
Qualifications
- Licensure: Active unencumbered Physical Therapist (PT) license in the state of Texas.
- Education: Graduate of an accredited Physical Therapy program (DPT preferred).
- Clinical Experience: 1 year of PT experience is preferred; home health experience is a plus but not required for candidates with strong clinical skills.
- Driver Requirements: Valid drivers license reliable transportation and active auto insurance.
- Compliance: BLS certification and ability to pass a background/drug screen.
