Our client is seeking a dedicated Home Health Physical Therapist to join their multidisciplinary team in Grants Pass. This role allows you to step out of the high-volume clinic setting and into the patients real-world environment where you can design truly functional recovery plans that make a measurable difference in their daily lives.
Key Responsibilities
- Comprehensive Assessments: Evaluate patient functional status mobility and safety within their home environment.
- Treatment Planning: Develop and implement individualized plans of care aimed at improving strength balance and independence.
- Functional Rehabilitation: Focus on real-world skills such as fall prevention gait training and home exercise program education.
- Collaboration: Coordinate with an interdisciplinary team (RNs OTs and Physicians) to ensure a holistic approach to patient recovery.
- Documentation: Utilize electronic medical records (EMR) to maintain timely and accurate progress notes in accordance with CMS regulations.
Qualifications
- Licensure: Active unencumbered Physical Therapist (PT) license in the state of Oregon.
- Education: Graduate of an accredited Physical Therapy program (DPT preferred).
- Certification: Current BLS (American Heart Association) required.
- Experience: 1 year of clinical PT experience preferred; home health experience is a major plus but not required for candidates with strong acute or rehab backgrounds.
- Mobility: Must have a valid drivers license reliable transportation and proof of auto insurance.
