A Day in the Life:
- Start your day visiting patients in their homes focusing on comfort dignity and quality of life
- Assess patient needs manage symptoms and adjust plans of care as conditions change
- Coordinate closely with physicians hospice aides LPNs social workers and chaplains
- Support and educate families helping them feel confident and cared for
- Document care clearly and efficiently in our EMR Consolo.
- Advocate for your patients while maintaining autonomy in how you structure your day
What Youll Be Doing:
- Serve as the primary nurse coordinating care for your assigned patients
- Develop implement and update individualized plans of care
- Determine visit frequency based on patient acuity and family needs
- Provide guidance to Hospice Aides and LPNs to ensure excellent care
- Communicate changes in condition to physicians and the care team
- Participate in on-call rotation
Background Youll Need:
- Active RN license in the state youll practice
- Graduate of an accredited nursing program (ADN or BSN)
- 1 year of RN experience required working for a hospice agency/program.
- Strong clinical judgment compassion and communication skills
- Reliable transportation valid drivers license auto insurance and CPR certification
