Program Overview
Third Street Supportive Housing Residence is a permanent supportive housing program established in 2007. Part of Henry Street Settlements Transitional & Supportive Housing Division the residence provides comprehensive on-site social services for 52 tenants. The program prioritizes the wellness and safety of both individual residents and the broader community.
Position Summary
The Case Manager provides comprehensive case management services to residents assisting them in accessing medical social and supportive services. This role involves direct client engagement individualized service planning advocacy and coordination with internal and external service providers to support tenants independence wellness and rehabilitation goals.
Qualifications
- Bachelors degree in social work or related health/social services discipline required.
- Minimum of two (2) years experience providing case management services to chronically mentally ill homeless or chemically dependent populations.
- Demonstrated experience in New York supportive housing particularly with special needs populations.
- Strong individual and group facilitation skills.
- Bilingual/bicultural (English/Spanish) preferred.
- Ability to work collaboratively as part of a multidisciplinary team.
- Flexibility to work occasional late shifts or adjust schedule as needed.
Key Responsibilities
- Provide individualized case management coordinating bio-psycho-social care and assisting tenants in accessing public benefits and community resources.
- Maintain accurate and up-to-date case records for each assigned tenant; develop measurable objective service plans aligned with regulatory guidelines.
- Gather and share resource information with tenants in a clear and accessible manner.
- Occasionally they accompany tenants to appointments emergency visits and other essential services.
- Report serious incidents allegations or sensitive situations immediately to supervisors.
- Facilitate direct services including individual and group activities.
- Network refer and coordinate care with other service providers including APS primary care physicians psychiatric services employment agencies substance use programs and home health aides.
- Provide crisis intervention and immediate support as needed.
- Perform other related duties as assigned by supervisor.
Essential Physical Requirements
- Ability to work onsite in Lower East Side NYC.
- Must be able to ascend and descend up to six flights of stairs as necessary.
