Job Summary
Nemours is seeking a full-time Pediatric Clinical Pharmacy Specialist (Hematology-Oncology) to join our team at the Lisa Dean Moseley Foundation Institute for Cancer and Blood Disorders at Nemours Childrens Hospital in Wilmington Delaware.
The Pediatric Hematology/Oncology Clinical Pharmacy Specialist provides advanced comprehensive pharmaceutical care to children and adolescents with hematologic and oncologic conditions in the ambulatory setting. This pharmacist will practice as an integral member of the multidisciplinary care team partnering closely with physicians advanced practice providers nurses and the pharmacy staff to ensure safe effective and evidencebased medication management.
The specialist will support chemotherapy management optimize supportive care participate in clinical research and quality improvement initiatives and serve as a preceptor for pharmacy students and residents.
Essential Functions:
- Review and interpret medication orders; assess for clinical appropriateness therapeutic goals adverse effects contraindications interactions and diseasespecific considerations.
- Evaluate monitor and modify drug therapy for pediatric hematology/oncology patients including chemotherapy and supportive care.
- Provide comprehensive medication counseling for patients and families including oral chemotherapy education and adherence support.
Serve as the clinical liaison between the pediatric oncology practice and the Specialty Pharmacy collaborating with pharmacy staff and support teams to ensure timely medication access and continuity of care
Advance standardized evidencebased care by developing and updating clinical guidelines pathways and EMRbased decision support.
- Participate in drug therapy consults and pediatric oncology clinical research protocols as needed.
- Support quality assurance and improvement efforts to uphold accreditations and ensure compliance with regulatory and institutional standards.
- Provide drug information and clinical recommendations to pediatric oncology providers nurses and other healthcare professionals.
- Precept pharmacy students and residents; contribute to educational programming within the pharmacy department.
- Representing Pharmacy Services on committees and workgroups as assigned.
Qualifications:
- PharmD with PGY-1 residency or 5 years pediatric hospital experience required.
- PGY-2 specialized residency training in pediatrics and/or Hematology-Oncology preferred.
- Experience in ambulatory oncology pharmacy practice preferred.
- Hospital pharmacy experience required (pediatric hospital pharmacy experiencepreferred).
- Active Pharmacist License (or legally eligible) in States of Delaware New Jersey & Pennsylvania required.
- Board Certification in Oncology Pharmacy Specialty (BCOP) required within 12 months of eligibility.
- American Heart Association BLS and PALS required upon hire. ACLS preferred.
- PA Clearances are not required for those associates hired to work in DE.
