Who Were Looking For
Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active Ohio nursing license
Required hospice nursing experience
Minimum 1 year of clinical hospice or home health experience
Strong understanding of patient care standards safety protocols and individualized care plans
Valid drivers license with reliable insured transportation
Compassionate professional and able to successfully pass a background check

Your Role & Responsibilities
Educate caregivers on company policies documentation requirements and hospice care standards
Support and reinforce hospice-directed plans of care with caregivers
Observe assess and communicate changes in patient condition to hospice care teams
Serve as a clinical liaison among patients families caregivers and leadership
Address questions and concerns with empathy while upholding high-quality patient care
Ensure accurate documentation and compliance with regulatory and hospice guidelines
Support and reinforce hospice-directed plans of care with caregivers
