We are searching for a talented Advanced Practice Practitioner to join our Health at Home Home Based Medical Care team.
Hours: Full-time - 40 hours8A 4:30P
Shift: Day no weekend holiday or on-call
Location: Fairhaven New Bedford MA

Responsibilities
- Under Direction of the Director of Transitional Care and or SVP Southcoast Health at Home and in collaboration with high-risk telehealth NP.
- The Nurse Practitioner (NP) or Physician Assistant (PA) provides home-based medical care to patients enrolled in the Remote Patient Monitoring (RPM) program with a primary focus on Medicare beneficiaries requiring face-to-face evaluations for RPM enrollment and ongoing clinical management.
- This role serves as a clinical resource to home health staff including nurses physical therapists occupational therapists and other interdisciplinary team members offering in-home consultations to support chronic disease management symptom escalation and urgent clinical needs for homebound patients.
- The NP/PA works collaboratively with primary care providers specialists home health teams and RPM clinicians to treat patients in place reduce avoidable emergency department utilization and improve outcomes for high-risk populations.
- This role combines clinical expertise patient education and a strong focus on improving outcomes in the outpatient settings.
Qualifications
- Requires a Master of Science Degree in Nursing/APRN or Masters degree in physician assistant studies from an accredited program.
- Proven ability to care for adult and geriatric patients with a broad range of physical and psychological problems is required.
- Proven ability to demonstrate maturity self-confidence and the ability to manage complex patient management plans with physician collaboration.
- Experience providing home-based care is required. Strong assessment and independent clinical decision-making skills.
- Minimum 2-3 years of clinical NP experience preferably in primary care geriatrics hospital medicine home-based care or chronic disease management.
- Nurse Practitioner: Requires a Master of Science Degree in Nursing.
- Certification from a national accrediting body to practice as a Nurse Practitioner is required.
- Current license to practice in the Commonwealth of Massachusetts as a Registered Nurse.
- Certification (of current eligibility) issued by the Commonwealth of MA and the State of Rhode Island to practice as a registered nurse is required.
- Physician Assistant: Graduate of an A.M.A. approved Physician Assistant program is required.
- Certification from a National accredited body to practice as a Physician Assistant is required. Must possess current CPR certification.
Pay Range
USD $59.41 - USD $71.45 /Hr.
