Concord Hospital Medical Group is seeking a compassionate and collaborative experiencedAdvanced Practice Provider (Nurse Practitioner or Physician Assistant)to join ourRheumatologyteam. This full-time position offers a balanced opportunity to practice in bothConcord and Laconia caring for patients across the Lakes Region and Capital Area of New Hampshire.
This role is ideal for an experienced APP who values patient education team-based care and developing long-term relationships with patients managing chronic rheumatologic conditions.
About the Role
As part of our Rheumatology practice you will care for patients with a wide range of autoimmune and inflammatory conditions including but not limited to rheumatoid arthritis lupus psoriatic arthritis gout and vasculitis. Youll work closely with experienced rheumatologists and a multidisciplinary care team to deliver high-quality coordinated care.
Key Responsibilities
Perform comprehensive patient evaluations including history physical exams and diagnostic assessments
Manage and monitor patients with chronic rheumatologic conditions
Develop and adjust treatment plans including medication management and monitoring
Review and interpret laboratory and imaging results
Provide patient and family education on disease processes medications and lifestyle considerations
Collaborate with physicians and other care team members to ensure seamless care
Participate in quality improvement initiatives and clinical program development
Qualifications
Licensed Nurse Practitioner or Physician Assistant in New Hampshire (or eligible for licensure)
Prior experience in rheumatology and internal medicine/primary care strongly preferred
Strong clinical judgment communication and organizational skills
Commitment to patient-centered care and education
