Starts at $36.90/hr will be more based on experience
BSN Required looking for 3-5 years Bedside/Homecare experience mixed with teaching experience
20 hours per week - days and times flexible; no weekends
Liaison will work at Main Campus but will travel to Homecare (255 E Main St) on occasion

Essential Functions:
- Assists in developing and monitoring appropriate home care needs for patients.
- Informs patient and family about plan status and changes based on progress. Monitors patient progress and identifies issues and concerns regarding progress. Works with multidisciplinary teams to create an overall patient care plan.
- Collaborates with Homecare staff care coordination and other stakeholders on patient needs prior to discharge home.
- Provides Family Centered Care. Promotes and provides necessary patient and caregiver education for the patients prescribed therapies.
- Initiates clear and purposeful written and verbal communications with the patient family and other members of the health care team to review nursing support patient/family response and progress towards discharge home. Documents according to organizational standards policies and procedures.
- Serves as a communication partner with the Homecare team to disseminate operational information between the hospital and Homecare.
Education Requirement:
BSN required.
Licensure Requirement:
Valid Ohio drivers license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection insurance eligibility driving qualifications and training set forth by Nationwide Childrens Hospital and maintain qualification of insurance guidelines.
Skills:
- Must have excellent critical thinking skills good communication and customer service skills and the ability to organize and manage time well.
- Ability to work independently as well as within a multi-disciplinary team.
- Pediatric Assessment Wound Care Infusion Central line Care Injections Patient Education.
Experience:
One year of Pediatric nursing experience required.
