Experience / Requirements

Masters degree
Nurse Practitioner with National Board Certification and active state license
1 year recent Primary Care NP experience preferred (or completion of CVS MinuteClinic training)
Knowledge of chronic disease management and quality measures
Strong communication organizational and problem-solving skills
Ability to work independently and collaboratively in a clinical team
Proficient with clinical technology and information systems
Willing to obtain multi-state licensure and work remotely when required

Duties / Responsibilities

Provide evidence-based primary and family care including assessment diagnosis and treatment
Manage patient health plans chronic conditions and preventive care
Perform routine exams and coordinate care with healthcare teams
Handle clinic operations patient follow-ups and administrative tasks
Educate patients on healthy lifestyle and wellness programs
