Youll collaborate with physicians therapists case managers and social workers to coordinate comprehensive care plans facilitate timely discharges and guide families through education and home care instructions. Youll document meticulously uphold strict infection control practices and participate in unit-based quality improvement initiatives to raise patient safety and outcomes.
