The Patient Navigator is a member of the inter-connected services established through the Massachusetts Department of Public Health/Bureau of Substance Addiction Services Hospital Based Substance Use Services Grant to improve access to and reduce utilization of acute services.Major Responsibilities:
* Identifying patient eligible for grant criteria via access through the Emergency Department
* Data collection and Entry
* Building community relationships with outside referral sources
* Providing reliable transportation for patients as needed
* Providing case management and referrals to meet patient needs
* Connecting patients to the UMass Harrington Healthcare Behavioral Health continuum of care
* Providing follow up contact with patients while part of the Bridge Clinic
Standard Staffing Level Responsibilities:

Position Qualifications:
1. Experience with substance use disorders and mental health disorders preferred or willingness to learn.
2. Excellent communication skills and interpersonal skills.
3. Attendance to various staff meetings in accordance with grant requirements and to ensure program staff involvement.
4. Valid drivers license and proof of insurance
5. Strong organizational skills
6. Willingness to work with a multi-disciplinary team.
License/Certification/Education:
Required: Bachelors Degree
Preferred: Certified Alcohol and Drug Counselor (CADC) or Licensed Drug and Alcohol Counselor (LADC) preferred
Experience/Skills:
Required: Experience with substance use disorders and mental health disorders preferred or willingness to learn.
Preferred: Two years working within the substance use disorders field
