Position Overview
The Care Coordinator II plays a key role in managing and optimizing care for patients undergoing joint replacement and spine surgery. This position partners closely with surgeons primary care providers interdisciplinary teams and family members to ensure coordinated evidencebased care across the full continuumfrom preoperative planning through postacute transitions.
This is an exempt position with 8hour day shifts with start times between 7:008:00 AM based on patient census.

Responsibilities:
Completes and documents a preoperative health assessment screening and perioperative history and assessment for patients scheduled for joint replacement or spine surgery according to established criteria.
Reviews clinical documentation and communicates findings with providers of the CSO Team 7S/7E and attending physician/anesthesia team and revises plan of care as needed based on findings.
Demonstrates skills and knowledge necessary to provide care to adolescent adult and geriatric populations including knowledge of growth and development the ability to obtain and interpret information to identify patient needs and to provide the appropriate care needed.
Assesses patient/family learning needs/barriers and carries out teaching measures and makes all appropriate referrals utilizing available resources.
Completes follow-up phone calls for patient discharge and reports on findings in electronic medical records contacting surgeons with questions or concerns.
Demonstrates ability to prioritize workflow to complete tasks.
Establishes methods that support reciprocal communication between physician offices lab services specialty services 7S/7Eand the CSO team.
Assesses patient post discharge care needs initiates plan of care and initiates inclusion of the necessary interdisciplinary team members.
Case Management duties prior to surgery include pre-referrals in Ensocare for Home Health Care (HHC) / infusion services lab draws durable medical equipment such as hospital beds walkers commodes home oxygen and transportation home
Inpatient includes:
Facilitates daily and as needed multidisciplinary rounds team and family meetings rounds daily on 7S/7E ensuring plan for discharge matches pre-operative assessment and making necessary changes as appropriate supportive of the joint/spine program to HHC/physical therapy inpatient in home or outpatient. Ongoing updates for 7S/7E include DME floor stock and communication tool used by charge nurse and Nurse Practitioner.
Case Management support post-discharge includes:
Scripted calls after discharge and appropriate follow-up with surgeons office to secure prescriptions needed and any follow-up appointments. . Care feedback coordinated with Program and Unit Manager as needed.
Participates in system-wide care management and other program activities as requested. This includes rounding on 7S joint replacement patients/7Eand CSA spine patients for discharge preparedness multi-disciplinary rounds and updating tracking forms.
Maintains collaborative professional working relationships with clinicians and administrative staff necessary to develop and implement a successful cross continuum care management for patients undergoing joint replacement /spine surgery.
Collaborates with Total Joint Replacement/Spine Program Manager on ongoing performance improvement initiatives.
Demonstrates understanding and application of CCHS values and behaviors in daily work.
Serves as a patient advocate when barriers impede progress and/or collaboration between practitioners involved in care that is fragmented.
Performs patient education via telephone or in person using teach back principals; collaborates with patient and family to establish realistic goals and activities to enhance patient self-management and participation in plan of care through discharge.
Use theoretical and educational frameworks and expert clinical knowledge base to promote develop and provide patient and family education.
Assumes responsibility for personal growth and is willing to share knowledge with co-workers management physicians and other healthcare providers.
Utilizes effective verbal and written communication skills with patients families co-workers management physicians and other members of the healthcare team.
EDUCATION AND EXPERIENCE REQUIREMENTS:
BSN is required. RN licensed to practice nursing in the state of Delaware
BLS required
Masters degree in nursing preferred
Three or more years of clinical experience in acute care or Peri anesthesia care required
Relevant direct orthopedic/spine experience as a professional nurse
Approved certification (by cost center) preferred
Hourly Pay Range: $41.28 - $66.05
This pay rate/range represents ChristianaCares good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills relevant experience and qualifications as they relate to specific job requirements.
