Job Summary
Under the clinical direction of the Nurse Case Manager and the administrative direction of the Aide Supervisor the CNA/HHA provides hands-on care to adult clients in their homes. This role focuses on personal care safety dignity and comfort - while maintaining accurate documentation in accordance with regulatory standards.
What Youll Do
Provide direct one-on-one care to clients in their homes
Assist with personal care such as bathing grooming dressing and toileting
Safely lift transfer and assist clients with mobility
Observe and report changes in client condition to the care team
Document care activities accurately and on time
Work flexible hours to support client needs (great for per-diem caregivers!)
What Were Looking For
Education
High School Diploma or GED required
Experience
1 year of healthcare or caregiving experience preferred
Certification
Home Health Aide (HHA):
Employed as a nurses aide within the past 24 months OR
Willing to pass an agency-approved competency test
Certified Nurse Aide (CNA):
Active Connecticut Nurse Aide Registration required
