Job Summary
Employment Type: Full-Time
Weekly Compensation Up To: $1540.00
Schedule: Day/Evening Rotation (5 x 8-hour shifts)
Weekend & Holiday Requirement: Every Other Weekend (EOW) and Holidays
Patient Population: Adult & Geriatric
Unit Type: Long-Term Care (LTC)
Position Overview
- Provide direct and indirect nursing care under the supervision of a Registered Nurse
- Collaborate with an interdisciplinary healthcare team to ensure safe effective and high-quality resident care
Key Responsibilities
- Provide direct and indirect skilled nursing care to assigned residents
- Collect and assess data related to residents physical and psychosocial conditions
- Communicate assessment findings to the RN and interdisciplinary team
- Contribute to the development and implementation of age-appropriate culturally competent plans of care
- Recommend revisions to plans of care based on resident outcomes
- Collaborate with residents families physicians and ancillary staff to promote optimal outcomes and satisfaction
- Maintain accurate and timely documentation in resident medical records
- Promote a safe healthy environment while considering quality safety and cost of care
- Participate in orientation mentoring continuing education and in-service programs
- Demonstrate leadership and accountability within the scope of LPN practice
- Comply with all applicable policies procedures and regulatory requirements
Required Qualifications
- Active New York State LPN License
- BLS Certification (AHA or RQI required)
- Minimum 1 year of experience as an LPN
- Minimum 1 year of experience in a Long-Term Care (LTC) setting
