Weekly Compensation Up To: $1540.00
Schedule: Day/Evening Rotation (5 x 8-hour shifts)
Weekend & Holiday Requirement: Every Other Weekend (EOW) and Holidays
Patient Population: Adult & Geriatric
Unit Type: Long-Term Care (LTC)

Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a full-time opportunity in Waterloo NY. This role is ideal for an LPN who demonstrates strong clinical skills professionalism and a commitment to patient-centered care in a long-term care environment.
  • Provide direct and indirect nursing care under the supervision of a Registered Nurse
  • Collaborate with an interdisciplinary healthcare team to ensure safe effective and high-quality resident care

Key Responsibilities

  • Provide direct and indirect skilled nursing care to assigned residents
  • Collect and assess data related to residents physical and psychosocial conditions
  • Communicate assessment findings to the RN and interdisciplinary team
  • Contribute to the development and implementation of age-appropriate culturally competent plans of care
  • Recommend revisions to plans of care based on resident outcomes
  • Collaborate with residents families physicians and ancillary staff to promote optimal outcomes and satisfaction
  • Maintain accurate and timely documentation in resident medical records
  • Promote a safe healthy environment while considering quality safety and cost of care
  • Participate in orientation mentoring continuing education and in-service programs
  • Demonstrate leadership and accountability within the scope of LPN practice
  • Comply with all applicable policies procedures and regulatory requirements

Required Qualifications

Licensure / Certification
  • Active New York State LPN License
  • BLS Certification (AHA or RQI required)
Experience
  • Minimum 1 year of experience as an LPN
  • Minimum 1 year of experience in a Long-Term Care (LTC) setting
