Job Summary and Responsibilities
The Health Coach is an important clinical participant of The GUIDE Model Program for patients living with
dementia. As a Health Coach at Yavapai Regional Medical Group you will play a crucial role in facilitating access
to local community resources for patients and their families to foster an improved dementia experience. You
will collaborate closely with community-based partner organizations and members to assess their health needs
offer education and support and advocate for their access to healthcare services. Acting as a bridge between
patients (participants/community members) and health/social service systems you will work alongside other
healthcare professionals to ensure the delivery of optimal care.
- Telephonic virtual and office based interactions with persons living with Dementia and their caregivers.
- Partners with clinicians to provide member care coordination advocacy health literacy and care planning within a clinical practice setting.
- Facilitates member coordination with provider/interdisciplinary team community partner organizations and
agencies that serve members living with dementia/their care partners and provides timely and accurate
reporting and communications. - Documents problems assessments and/or interventions and promotes ease of access to a continuum of care through appropriate information and referral.
Job Requirements
High School Graduate
Preferred
1-3 years Social Work Dementia Care Care Coordination Community Health and/or Medicare Experience
