Job Overview:
We are seeking a compassionate and experienced Registered Nurse to join our Home Health and Hospice team as a Hospice RN Case Manager. This full-time role focuses on managing hospice patients in the home and senior living settings providing coordinated patient-centered care throughout the end-of-life journey. The Hospice Case Manager oversees assigned patient caseloads collaborates with interdisciplinary teams and ensures continuity of care. This position offers stability and longevity with a well-established hospice organization and a supportive mission-driven culture.
Coverage Details:
- Coverage Area: North Kansas City including Liberty Platte City Excelsior Springs Blue Springs and Independence
- Average Daily Mileage: Approximately 50 to 100 miles per day typically 5070 miles
- Mileage Reimbursement: $0.45 per mile excluding mileage to and from home
Key Responsibilities:
- Manage and coordinate hospice patient caseloads in home and senior living settings
- Conduct comprehensive patient assessments and ongoing evaluations
- Develop implement and update individualized hospice plans of care
- Provide skilled nursing care and symptom management for hospice patients
- Collaborate with physicians social workers chaplains and other interdisciplinary team members
- Educate patients and families on disease progression comfort measures and care plans
- Maintain accurate and timely documentation using electronic charting systems
- Travel between patient locations throughout the service area as part of daily responsibilities
- Ensure compliance with hospice regulations standards of care and organizational policies
Required Qualifications:
- Active Registered Nurse (RN) license in Missouri and/or Kansas as applicable
- Prior hospice experience required
- U.S. citizenship or valid green card
- Strong clinical assessment case management and time management skills
- Ability to work independently in a home-based care environment
- Excellent communication and interpersonal skills
