Job Summary

POSITION SUMMARY:

The Case Manager works directly with those released from jail establishing trust assessment service plan and follow-up post release creating a successful reentry plan.

This is a lived experience position providing shared case-management of 85 faith-based reentry clients in a professional team setting working in partnership with the Diversion Reentry Service (DRS) center. Staff a resource table at the Elmwood Jail every other Monday evening 7:30PM 10:00PM and one Friday a month. The rest of the hours are in- person at John XXIII Multi Service Center M-F flexed 9:00AM 5:00PM.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide paraprofessional court representation on behalf of clients. Create a trauma informed linguistic culturally competent safe and welcoming environment.

  • Works directly with the criminal justice population the Department of Corrections (DOC) local service providers to plan for reintegration from county jail into the community.

  • Communicates with Probation Parole San Jose and South County DRS centers including Outreach Court Superior Court the District Attorney (DA) Office and Public Defender (PD) Office.

  • Administer client needs assessments create a service plan develop client budgets provide service navigation coordination of behavioral health referrals to DRS including educational employment & training programs; coordinate pro- social networks and enter data into AWARDS HMIS and Santa Clara County referral tracking system (RTS).

  • Secure housing through the Emergency Assistance Program (EAP) applications to the Office of Supportive Housing.

  • Maintain good communication with approved Sober Living Environments (SLEs) and court ordered substance use disorder (SUDs) housing.

  • Participate in quarterly faith-based collaboration monthly DRS brown bag resource meetings monthly staff and agency all staff meetings.

  • Participate in events and special projects assigned by the DRS center and CCSCC.

  • Track client gift card and bus token logs. Submit check requests on SignNow to support client Tattoo removal Domestic Violence (DV) classes Court ordered DUI driving classes Housing Education Union Dues Car Repair DMV Car Insurance Cell Phone Bills Transportation Work Clothes Work Tools Dental Bills Behavioral Health Counseling Food and Hygiene.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

  • Minimum of three (3) years of experience and/or Peer Mentor Certification working with reentry population serving homeless populations providing service to diverse high needs populations.

REQUIRED KNOWLEDGE SKILLS AND ABILITIES

  • Maintain trainings on trauma informed crises intervention VISPDAT HMIS SUDS Behavioral Health.
  • Excellent skill and judgment in providing direct services to clients.
  • Excellent interpersonal oral and written communication skills.
  • Knowledge of community culture attitudes services and resources for formerly incarcerated adults and associated ethnic communities.
  • Demonstrated computer skills with Microsoft products and web-based applications.
  • Bilingual in English/Spanish skills are beneficial but not required.
  • Flexibility to adjust work hours and assignments.

PHYSICAL REQUIREMENTS

  • Ability to work at a desk for extended periods of time.
  • Ability to use a computer workstation.
  • Ability to lift up to 25 lbs.
  • Ability to travel to jail sites.

OTHER QUALIFICATIONS:

  • Criminal background check via livescan fingerprint
  • Must have TB test performed and submit results
  • Provide proof of COVID-19 vaccination and booster status
  • Automobile valid drivers license and auto insurance per agency policy or have access to reliable transportation

WORKING CONDITIONS:

  • Work will be mostly engaging with clients at the jail site.
  • Some time will be spent driving between the jail site and CCSCC facilities.

HOURS AND OTHER CONDITIONS:

  • Standard office hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.
  • This is a full-time non-exempt position. Hours of operation will be Mon-Fri 9:00AM to 5:00PM. Every other Monday and one Friday a month inside Elmwood jail 7:30PM 10:30PM.
