Inabia is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join its Skilled Nursing team. This role provides direct nursing care to residents in a skilled nursing facility ensuring high-quality patient-centered care during night and mid/night shifts.
The ideal candidate is dependable clinically competent and comfortable working extended and overnight shifts in a long-term care environment.
Key Responsibilities- Performs initial data collection on all residents and follows up on date collection per GSLS policies and procedures. Administers tracks progress and checks activities related to residents.
- Medication Administration: . Observes documents and reports resident responses to medications. Is knowledgeable and follows medication policies and procedures.
- Competently performs skin treatments catheterizations tube feedings and other treatments and cares as needed.
- Documentation: Documents professionally accurately and completely according to current standards and GSLS policies and procedures.
- Coordination/Direction: Coordinates and directs resident care appropriately and professionally-assumes responsibilities for all nursing care and nursing actions to resident on the shift assigned. Communicates pertinent resident information to the appropriate staff family and physician.
- Emergency and Disaster Measures: Carries out first aide emergency and disaster measures according to policies procedures and standards of care.
- Awareness of resident employee and public safety always. Recognize report and/or remove safety hazards (environmental equipment conditions and practices).
- Other duties within their scope of practice as directed by the registered nurse or Team Leader.
Physical Demands of an LPN (Licensed Practical Nurse):- Regularly required to stand bend and walk. May also be required to sit climb stoop kneel crouch push and pull.
- Regularly assigned to lift or move objects up to 10 pounds.
- Frequently required to support up to 10-50 pounds.
- Emergency situations could require carrying/supporting 25 pounds or greater.
- Requires employee use hands for repetitive actions such as simple grasping firm grasping and fine manipulating.
- May be required to work in unprotected heights being around moving machinery exposure to marked changes in temperature and/or humidity driving automotive equipment and exposure to dust fumes or gases.
- Requires functional hearing ability to read medical records make observatory assessments and document in writing and computer records.
- Requires functional ability to perform job duties as listed in the job description
Schedule & Workload- Guaranteed Hours: 40 hours per week
- Shifts:
- (3) 8-hour night shifts
- (1) 16-hour mid/night shift
Qualifications- Graduate of a licensed practical nurse accredited program required.
- Current active LPN nursing license in the State of Minnesota in good standing.
- CPR certified.
- Must be able to read write and communicate in English.
- Clinical experience with geriatrics long-term care and/or rehabilitation preferred.
- Demonstrates a broad and current knowledge of nursing including gerontological nursing and related aspects such as rehabilitation and metal health.
- Knowledge of and skilled in physical assessment techniques to gather data.
- Skilled in oral and written English communication-communicates clear expectations
