Description

The Childrens Behavioral Health Home (BHH) Coordinator / Targeted Case Manager is responsible for overseeing and coordinating comprehensive care services for children and families with behavioral health needs.


This full-time (4 out of 5 days per week) role blends direct client support with care coordination documentation advocacy and collaboration with multidisciplinary teams to ensure high-quality person-centered care.


The ideal candidate is compassionate organized and skilled at engaging children and families including those who may be hard to reach or hard to engage while navigating complex systems of care.


Essential Duties & Responsibilities

Client Assessment & Treatment Planning

  • Establish and maintain therapeutic relationships with clients and families consistent with program philosophy and values
  • Develop individualized treatment plans in collaboration with families guardians and service providers
  • Monitor and update treatment plans to reflect changing needs
  • Prepare comprehensive treatment plans quarterly updates discharge plans and required documentation
  • Support families in identifying goals and coordinating services for their children

Case Management

  • Provide integrated case management by assessing strengths and needs coordinating services and monitoring progress
  • Lead treatment teams and facilitate quarterly treatment team meetings (or as clinically indicated)
  • Participate in safety planning and crisis intervention; provide enhanced access during evenings and weekends as needed
  • Ensure compliance with all regulatory program and documentation standards

Coordination of Services

  • Collaborate with community partners including mental health providers hospitals residential facilities schools and social service agencies
  • Educate families about behavioral health resources and community supports
  • Ensure timely access to services for all referrals
  • Advocate for children and families within education healthcare and social service systems

Team Collaboration

  • Attend weekly BHH team meetings and monthly staff meetings
  • Participate in clinical reviews and professional development opportunities
  • Maintain productivity expectations and assist with program development initiatives

General Expectations

  • Maintain confidentiality of client and agency information
  • Foster a welcoming inclusive environment for individuals seeking services
  • Work effectively with individuals from diverse backgrounds and abilities
  • Demonstrate flexibility to meet client and organizational needs including occasional overtime

Physical & Work Requirements

  • Ability to sit stand walk and operate standard office equipment
  • Ability to lift up to 10 pounds occasionally
  • Work frequently occurs in home and community settings that may be noisy or unpredictable
  • More than 50% of time is spent in community-based settings
  • Reliable transportation required; travel within the catchment area is expected

Schedule & Compensation

  • MondayFriday 8:00 a.m. 5:00 p.m.
  • Enhanced access requires availability during evenings and weekends based on client needs
  • Compensation follows Wage and Hour guidelines under the Fair Labor Standards Act

Requirements

The position requires a bachelors degree with a specialization in psychology mental health human services behavioral health behavioral sciences social work human development special education counseling rehabilitation sociology nursing or a closely related field.

OR have a bachelors degree from an accredited four (4) year educational institution in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience.

  • Must complete the Child and Adolescent Needs and Strengths (CANS) assessment tool and pass the exam annually.
  • Must possess a valid drivers license and reliable transportation.
