Responsibilities
Located on the Community Regional campus Burnett Extended Care Center is our brand-new 154-bed transitional care facility opening this summer. Purpose-built to serve patients transitioning from hospital to home Burnett includes specialized units Memory Care Long-Term Care and Transitional Care that focus on quality comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative post-acute setting.
- Assist residents with activities of daily living such as bathing dressing grooming toileting and mobility support.
- Support RNs and LVNs by observing residents documenting care and promptly reporting any changes in condition.
- Deliver direct hands-on care to residents requiring 24-hour medical supervision ensuring safety and comfort throughout their recovery or long-term stay.

Qualifications
Education & Experience
- High School Diploma High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
- Certified Nursing Assistant training to obtain certification required
Licenses & Certifications
- CNA - State of California Certified Nursing Assistant Certification required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
