Job Summary

Summary

The Patient Care Coordinator (PCC) serves as the Nursing supervisor during non-administrative work hours including nights evenings weekends and holidays. The PCC works with the Bed Management Center to coordinate all admissions and various patient needs such as Direct Observation agency personnel and troubleshooting issues during the off-tours. This PCC RN is primarily responsible for enforcing the medical centers policy procedures and directives while supervising nursing personnel.


Duties

The Patient Care Coordinator (PCC) assumes authority responsibility and accountability for managing and directing employees staffing units and allocating resources as necessary during weekends holidays and after-hours tours during the absence of nursing management/leadership. The PCC is responsible for rounding and is a consultant to providers. Collaborates with the Administrative Officer of the Day (AOD) and other members of the interdisciplinary team as needed for decisions related to on-going patient care and actions taken in emergency situations and Functional Statement: Nurse II Last Updated: January 292024 Page 2 disasters. The PCC provides programmatic outcomes to nursing leadership to assist with improving organizational processes and the delivery of patient care.

Duties include but are not limited to the following:

  • Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice.
  • Assists the Nurse Managers by contributing to performance evaluations educational development for the non-administrative tour employees and meeting performance measures.
  • Applies clinical knowledge/judgment to lead staff involvement in planning decision making and evaluating outcomes within the immediate practice setting.
  • Effectively functions as a leader with demonstrated outcomes for the immediate practice setting.
  • Proactive in implementing policies and procedures (including medication administration and restraint and seclusion) at the program/service level and monitors compliance by staff members. Initiates and implements corrective actions as indicated at the program/service level.
  • Utilizes the team approach to implement patient safety goals.
  • Supports discharge panning and continuity of care by supervising staff as they develop an individualized plan of care in collaboration with the patient and health care team. Involve community resources as appropriate.
  • Improves Veteran/patient driven care by resolving experience issues such as coordination of care timeliness or process issues common themes of dissatisfaction or environment of care concerns for the immediate practice setting.
  • Applies patient family and caregiver knowledge values and beliefs into care delivery without judgement or discrimination.
  • Serves as an advocate for patients families and caregivers improving outcomes for the immediate practice setting.
  • Serves as a resource for patients caregivers and staff to address and resolve ethical issues in the immediate practice setting.
  • Leads the implementation of high reliability principles and I CARE values to increase consistency of care delivered and improve patient outcomes.
  • Effectively sets group goals and keeps self and team informed of the latest trends and developments. Inspires the cooperation and confidence of others. Demonstrates a high level of expertise.

  1. BSN.
  2. Evidence of progressive leadership duties (charge nurse duties team leader etc.)

The Dimension requirements (Practice Veteran/Patient Driven Care Leadership Professional Development and Evidence-Based Practice/Research) are detailed for each grade and level within the online assessment: I Level IDelivers fundamental knowledge-based care to assigned clients while developing technical competencies.An Associate Degree (ADN) or Diploma in Nursing with no additional professional nursing required.Nurse I Level IIDemonstrates integration of biopsychosocial concepts cognitive skills and technically competent practice in providing care to clients with basic or complex.An ADN or Diploma in Nursing AND 1 year of specialized nursing experience equivalent to Nurse I Level 1 ;OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.Nurse I Level IIIDemonstrates proficiency in practice based on conscious and deliberate planning.
