The Case Management Registered Nurse plays a key role in supporting Veterans through coordinated patient-centered care across the continuum. This position focuses on assessment care planning and collaboration to address complex medical psychosocial and transitional needs. Working closely with interdisciplinary teams the nurse helps Veterans and their caregivers navigate treatment decisions access appropriate services and achieve stability and improved quality of life. This role is ideal for a nurse who values holistic care relationship-building and meaningful advocacy for Veterans and families.
Major Duties include but are not limited to:
Conduct age-appropriate assessments to identify medical psychosocial and support needs that impact treatment and recovery.
Guide Veterans and caregivers through available care options services and resources to support informed decision-making and self-management.
Collaborate with interdisciplinary teams to develop implement and evaluate individualized plans of care and discharge strategies.
Facilitate safe and timely transitions of care by coordinating services follow-up appointments and community resources.
Monitor care outcomes and service delivery to ensure timely appropriate and Veteran-centered provision of care.
Advocate for Veterans by identifying barriers to care and coordinating solutions that support continuity and quality outcomes.
Communicate effectively with providers Veterans and caregivers to reduce fragmentation and promote a seamless care experience.
