As a vital member of our interdisciplinary care team you will:
Promote Elara Carings mission philosophy and standards of excellence
Assist in implementing individualized rehabilitation plans under the direction of an Occupational Therapist
Perform routine patient visits and reassess therapy needs at each visit
Communicate significant findings or changes in patient condition to the OT physician and care team
Verify and follow the established Plan of Care and physician orders
Educate patients and caregivers on therapy goals home exercise programs and safety
Ensure compliance with Medicare Medicaid and third-party payer requirements
Identify and report potential coverage or payment concerns
Maintain timely accurate clinical documentation
Collaborate with clinical staff on schedules patient needs and care coordination
Support initiatives to prevent avoidable hospitalizations and ER visits
Work Environment & Physical Requirements
Physically demanding fast-paced clinical environment
Full range of motion required including lifting transferring and assisting patients
Ability to lift 50100 lbs
May require irregular hours or on-call responsibilities as applicable
What Is Required
Graduate of an accredited Occupational Therapy Assistant program
Current unrestricted COTA license in the state of practice
Minimum 1 year of clinical experience as a COTA
Ability and willingness to travel within the branch coverage area (up to 50%)
Valid drivers license reliable vehicle and current auto insurance
Ability to sit stand bend lift and move throughout the workday
This position reports to the Branch Director Clinical Manager Clinical Supervisor or designee.
