This position is located at The Martinsburg Veterans Affairs Medical Center in Martinsburg West Virginia. The CLC Admission Coordinator Registered Nurse (RN) is an essential role within our organization providing critical support in the admission transition and care of Veterans within the CLC.
The coordinators responsibilities in leadership collaboration assessment and data management are indispensable for maintaining high standards of care and optimizing resource utilization in the CLC. The coordinator is a subject matter expert in eligibility co-payment 10-10EZ & 10-10EZR updates means testing and coordinating with Acute Care services for Patient Flow/Difficult Discharge and care planning processes.
The positions responsibilities are foundational to maintaining high standards of care fostering collaboration and ensuring efficient and effective processes that are integral to the mission of our healthcare system
