This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval award amount (up to $200000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Provides and/or supervises direct nursing care in Rehab and Extended Care (REC) applying the nursing process to assess plan implement and evaluate age-specific care. Coordinates care and transitions educates patients/families administers medications and procedures and collaborates with the interdisciplinary team and quality improvement.
Provide and/or supervise direct patient care and be accountable for all elements of the nursing process.
Assess plan implement and evaluate care based on age-specific components.
Coordinate care focused on patient transition through the continuum of care and patient self-management after discharge.
Provide patient and family education.
Work with the care team to sign off and ensure completion of all orders for assigned patients and teammates.
Administer medications and perform procedures per established policies and guidelines.
Complete daily and weekly skin assessments and monitor/document pain and pain interventions on a daily and weekly basis.
Check crash carts for readiness and respond to codes as part of the code team as needed.
Collaborate with members of the interdisciplinary team and participate in unit/program level quality improvement.
Protect sensitive printed and electronic data and report information security and privacy incidents/violations.
