Duties/Responsibilities:
Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role all employees are expected to perform related tasks as needed. - Provide comprehensive eye exams refractions and medical eye care for acute and chronic ocular conditions.
- Diagnose and manage ocular diseases such as glaucoma diabetic retinopathy and infections.
- Participate in integrated primary care workflows coordinating services with medical dental behavioral health and case management teams.
- Document patient encounters in accordance with FQHC guidelines including Prospective Payment System (PPS) encounter requirements.
- Provide patient education regarding eye health vision conditions preventative care and treatment plans.
- Refer patients to ophthalmology or external specialists when needed.
- Engage in outreach activities as applicable to clinic programming.
- Support the mission of serving underserved and highâneed communities through equitable accessible care.
- Other duties as assigned.
Physical Requirements:
- Occupational Exposure Risk Classification: The normal work routine involves moderate exposure to blood body fluids or tissues.
- The normal work routine has a frequent ergonomic risk for repetitive motion impairment.
- The position may require a moderate amount of physical exertion (bending lifting twisting) in the work area.
Required Qualifications:
- Education:Doctor of Optometry (OD) from an Accreditation Council on Optometric Educationaccredited program required.
- Experience: Experience working with underserved or highâneed populations preferred.
- Certificate or Professional License:
- Current unrestricted and valid optometry license to practice in Indiana.
- CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
- BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
- Valid drivers license and active auto insurance coverage.
- Satisfactory results from criminal and driving record background checks
