Your role centers on delivering comprehensive physical therapy services: conducting patient assessments designing individualized treatment plans implementing evidence-based interventions and educating patients and families on home exercises and fall prevention. Youll monitor progress adjust plans for optimal outcomes document care in a timely compliant manner and collaborate with physicians nurses and therapy assistants to ensure seamless care transitions.
