Position Summary
The Community Living Supervisor coordinates the daily operation of assigned Community Home(s). Together with each person their family and other team members you will assist in charting a course that supports each person in realizing their life of personal significance. Responsibilities include supporting individual outcomes coaching staff and serving as the on-call contact based on a rotating schedule. This position reports to the Community Living Specialist.

- Receive a pay rate increase ($1/hr) and a one-time bonus upon completion.
Key Responsibilities
- Provide direct care supports to people with intellectual or developmental disabilities and specialized care in daily living social and recreational activities.
- Ensure compliance with Policies and Procedures relative to Community Homes; assemble maintain and monitor all records.
- Assist with and ensure the upkeep and maintenance of the home.
- Implements formal and informal outcome plans to assist the person in achieving their personal outcomes.
- Develops the weekly staff schedule and ensures all shifts are covered (adjusting for vacation or illness with approval).
- Communicates and resolves program and individual concerns with the Community Homes Supervisor; participates in staff meetings.
- Assign and ensure completion of each persons Annual Assessment and ensure the implementation of Individual Support Plan (ISP) outcomes.
- Distributes individual funds obtains receipts and assists with banking as advised.
- Administer medications (upon completion of provided certification training).
- Performs other duties as assigned.
Qualifications
Educational Requirement: Preferred Bachelors degree or equivalent with some previous experience.
Flexibility: Must be flexible to work on call as needed
Skills & Abilities: Ability to complete one-person physical transfers.
Certification Requirements: Must obtain CPR certification within 6 months of start date and pass Medical Administration Training (provided by Achieva).
Clearance Requirements: Must meet Act 33/34 clearance requirements.
License & Travel: Valid Pennsylvania drivers license required.
