Advanced Practice Provider-Adolescent Unit-Days
Role Overview:
An adolescent advanced practice provider is needed for the day shift on our adolescent crisis unit as we endeavor to build the healthiest community in the nation one person at a time. This position will be based at the new and innovative acute care behavioral health facility which opened in late 2023. SummitStone has the privilege of partnering closely with Larimer County on this venture. We provide comprehensive and collaborative care to individuals in crisis with mental health and/or substance use disorders.
Our adolescent APP will provide specialized care for adolescents 12-17 with behavioral health conditions inclusive of mental health and substance use our Adolescent Crisis Stabilization Unit the medical provider will collaborate with a multidisciplinary team to assess treat and support young patients in managing conditions like depression anxiety bipolar disorder suicidal ideation substance use or trauma related disorders. This role involves working closely with young patients their families and healthcare teams to promote emotional well-being manage mental health conditions and support recovery.
