Job Summary

Description

The Physical Therapist PRN is responsible for evaluating/assessing needs of referred patients and formulating treatment plans. Provides therapy services defined in treatment plans. Works cooperatively with physicians case managers and other members of the healthcare team. Maintains accurate documentation of therapy services in the medical record. May supervise physical therapy assistants therapy techs and students.



Responsibilities
  1. Patient Care: This will have been satisfactorily performed when
    1. Evaluation and treatments are appropriate for diagnosis age group patients physical and mental status and are performed and accurately documented for each patient as evidenced by chart review.
    2. A Plan of Care which contains; functional assessment treatment plan frequency/ duration of care timed/measurable Goals and a prognosis or expected end of care.
    3. Evaluations are completed and documented within 48 hours of the patients visit per chart review.
    4. Patient care is documented within 24 hours of the patients visit utilizing approved hospital and departmental abbreviations reflecting patient response to treatment progress towards goals and any adverse reactions to care per chart review.
    5. Patient care is conducted in a safe and effective manner as evidenced by no more than one incident report per year.
    6. Discharge documentation is completed within 48 hours of the patients last visit or upon notification from other qualified sources per chart review.
  2. Regulatory/Quality: This will have been satisfactorily performed when
    1. The patients pain rating is documented each visit with treatment modifications documented to reflect pain control measures when necessary per chart review.
    2. Patient Care documentation is recorded in timed intervals and corresponds to charged billable units as evidenced by chart reviews.
    3. Patient Charge sheets are completed and placed in the Charge Sheet Box within 24 hours of the date of the charges.
    4. Actively participates in at least one Quality initiative activity per year.
    5. The therapist will supervise report and mentor PT and PTA students as needed.
  3. Interdisciplinary Teamwork: This will have been satisfactorily performed when
    1. Ancillary and support staff is utilized as necessary and allowable to maximize productivity and enhance patient care. Appropriately delegates to and supervises performance of support personnel according to their level of training and in accordance with state law.
    2. The Therapist will work with patients co-workers and others to schedule patients in a timely and efficient manner.
    3. Any necessary information concerns complaints or suggestions are communicated utilizing the appropriate chain of command at all times.
    4. The Therapist will mentor educate and support co-workers and others as necessary.
  4. Other Duties: This will have been satisfactorily performed when
    1. The Employee participates in at least one of the following: hospital committee department process special project staff Inservice or other activity approved by the supervisor.
    2. Other duties as assigned are completed.


Qualifications
  • Experience - None
  • Education - Graduate of accredited Physical Therapy Program (minimum bachelors degree Degree)
  • Training - None
  • Special Skills - Good interpersonal/human relations skills; Good oral/written communication skills; Good teamwork skills; Good critical thinking/decision making skills; Superior organizational skills
  • Licensure - Current Louisiana State Physical Therapy License (temporary licensure allowed) and BLS Certification
