Position Summary: The Physical Therapist provides specialized physical therapy services within the home health and hospice environment delivering comprehensive patient-centered care while ensuring accuracy safety and compliance with all regulatory standards. This role provides rehabilitative services to home health and hospice patients carrying out skills that support treatment rehabilitation safety maintenance and education for patients and their families with particular attention to the geriatric population. Working as part of a collaborative interdisciplinary care team the Physical Therapist plans and coordinates therapy services for individuals and families assesses health needs and potential or existing hazards and consults with or refers patients and families to appropriate agency or community resources as needed. Treatment and preventive procedures are delivered under medical direction and care is evaluated planned implemented supervised and documented under the guidance of the supervisor or manager in accordance with established standards of performance and organizational policies. Key Responsibilities: Perform comprehensive physical therapy assessments and develop individualized evidence-based treatment plans. Provide therapeutic interventions focused on mobility strength pain reduction and functional independence. Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning. Educate patients and caregivers on safe mobility techniques equipment use and home-exercise programs. Document evaluations progress notes and care plans accurately and promptly in the electronic record.
Licenses and Certifications Required Licensed Physical Therapist in North Dakota. Current drivers license required and car available to travel to patients home. Educational Requirements Experience Requirements Strongly prefer 2 years previous experience as a Physical Therapist. Special Skills or Training Requirements Organize time well and meet job expectations and performance standards. Exhibits responsibility and flexibility in work situations willing to work irregular and/or less favorable hours. Demonstrates tact sensitivity sound judgment and professional attitude when relating with patients families and colleagues. Meet the physical and emotional demand of the position. Demonstrate a caring and respectful attitude when providing patient care. Physical Requirements: The role requires mobility throughout home or office settings prolonged periods of computer use or direct patient care and the ability to lift move or support 25-50 pounds depending on patient needs. The employee must maintain adequate vision hearing and communication abilities to safely perform all duties. Environmental Requirements: Work is performed in patient homes office settings and community environments depending on position type. Exposure to varying home conditions weather during travel infectious agents and equipment-related hazards may occur.
