Position Summary
Seeking an RN with strong care management and utilization review experience to support patient flow and safe discharge processes within the Emergency Department.

Key Functions

Perform utilization management and care coordination
Support discharge planning and transition services
Identify barriers impacting patient throughput
Communicate with care teams and payers
Maintain accurate clinical documentation

Requirements

Active NY RN license
Minimum 5 years RN experience
Experience in UR care coordination and discharge planning
ASN/ADN required
BLS certification

Preferred

BSN/MSN
Case Management certification
