Job Summary
Position Summary: The Occupational Therapist provides comprehensive evaluation and treatment services to help patients regain or improve functional abilities following illness injury or changes in health status. This role conducts thorough assessments establishes individualized treatment goals and delivers evidence-based interventions designed to maximize independence safety and quality of life. Working collaboratively with physicians nurses rehabilitation team members patients and families the Occupational Therapist contributes to coordinated patient-centered care across the hospital and medical district. This position functions within a supportive interdisciplinary rehabilitation environment that values teamwork communication and professional collaboration. The role also supports departmental quality initiatives documentation standards and ongoing professional development activities in alignment with Trinity Health standards. Key Responsibilities: Evaluate patients functional status home environments and safety risks to develop individualized occupational therapy plans of care. Provide therapeutic interventions focused on activities of daily living functional mobility adaptive techniques and environmental modifications. Educate patients and families on safety strategies equipment use energy conservation and functional maintenance appropriate to the care setting. Coordinate therapy services with nursing physicians and interdisciplinary team members to support integrated and effective care delivery. Maintain accurate timely and compliant documentation of evaluations interventions and patient progress in accordance with organizational and regulatory standards. Participate in care planning communication and quality activities to support continuity of care and positive patient outcomes.
Licenses and Certifications Required Licensed Occupational Therapist in North Dakota. Educational Requirements Experience Requirements 2 years previous experience as an Occupational Therapist strongly preferred. Special Skills or Training Requirements Organize time well and meet job expectations and performance standards. Exhibits responsibility and flexibility in work situations willing to work irregular and/or less favorable hours. Demonstrates tact sensitivity sound judgment and profes- sional attitude when relating with patients families and colleagues. Meet the physical and emotional demand of the position. Demonstrate a caring and respectful attitude when providing patient care. Core Competencies Competencies unique to Home Health include documentation understanding of regulations OASIS accuracy and competency in drug medication review when only discipline in the home. Physical Requirements: This position requires frequent travel to patient homes and involves standing walking bending reaching and prolonged activity during patient visits. The Occupational Therapist must be able to assist patients with functional tasks move therapy equipment and access home environments that may present physical challenges. Adequate vision hearing communication skills and physical stamina are required to safely assess patients provide instruction and complete documentation. Environmental Requirements: The Occupational Therapist works primarily in patient homes and residential care settings with varying layouts and environmental conditions. The role may involve exposure to infectious diseases household hazards and emotionally sensitive situations particularly in hospice care. Adherence to infection prevention practices safety protocols documentation standards confidentiality requirements and appropriate use of personal protective equipment is required at all times.
