DUTIES/RESPONSIBILITIES-
- Complete screening brief and comprehensive assessments develop differential diagnoses and formulate and implement treatment plans
- Provide therapeutic consultation to patients
- Provide behavioral health consultation to primary care medical and other health center staff
- Triage and respond to center-based behavioral health crisis intervention and management
- Obtain and document social family and psychological history
- Conduct child youth and/or adult individual group and family psychotherapy
- Complete progress notes and a detailed summary upon completion of treatment for each patient
- Provide behavioral management strategies and training for various chronic patient groups (e.g. diabetes compliance)
- Provide personal enhancement/ counseling
- Supervise graduate students and unlicensed professionals
- Utilize electronic health record to capture pertinent documentation
- Performs all other duties as assigned by the direct supervisor; undertakes special projects and program efforts as requested
- Investigates evaluates and monitors individual cases.
- Assists in rendering diagnosis and developing individual treatment/service plans designed to prevent or alleviate personal or environmental problems of patients/clients.
- Provides crisis intervention and individual or group counseling to patients/clients and their families.
- Maintains accurate records of patient/client activities.
- Attends and participates in departmental meetings and professional conferences as necessary.
- Participates in student training as a preceptor as needed within the department.
OTHER KNOWLEDGE/SKILLS/ABILITIES-
- Ability to interact with cultural competence with medically underserved groups and communities
- Skilled in Microsoft Office Suite including Word Excel and PowerPoint
- Must be organized and demonstrate initiative judgement problem-solving critical thinking and decision-making skills
- Must possess effective professional oral and written communication skills
- Must maintain an effective and positive professional working relationship with staff and patients
- Fast-paced inner-city medical office environment
- Occasional extended work hours with evening work (finish by 7pm).
- Commitment to NSCHC faith-based mission to provide care for all who request it without judgement.
QUALIFICATIONS (Required for Employment)
- Successful completion of a Masters Degree or above from an accredited university in the field of Social Work Professional Counseling Marriage Family Therapy or Psychology.
- Current active applicable professional licensure to practice as a mental health therapist in the State of Pennsylvania; LCSW required.
- Minimum two years of experience working with indigent medically underserved ethnic minority and/or international communities. Language expertise preferred but not required in one or more languages/dialects served by the health center.
- Experience in addiction trauma grief and/or adolescent therapeutic services.
- Demonstrated behavioral health program coordination clinical research and/or community project management experience.
- Evidence of current ability to perform privileges requested is required of all applicants for renewal of privileges
