We are seeking an Administration Assistant to provide collaborative administration support for the Healthcare at Home team in Geelong.
In this role you will primarily be responsible for Patient Administration consisting of clerical functions regarding patient admissions through to patient discharge. Additionally you will also provide clerical support to internal and external stakeholders of Healthcare at Home.
We are offering a casual position and a great opportunity to join a service dedicated in providing excellent standards of care to patients throughout the community.
The Position
Collaborates with the clinical teams to support the seamless delivery of patient care.
Responds to patient queries with appropriate escalation to clinical resources.
Compilation and management of the patient file including patient admission documentation care plans medications charts and clinical notes in accordance with the National Standards.
Processes patient referrals including scheduling visits to ensure appropriate allocation of resources giving consideration to skills and geographical location to optimize efficiencies.
Provides accurate data entry of patient information using the Patient Management System in alignment with procedures.
We are looking for a self-starter with a strong work ethic who can prioritise and multitask and will provide the highest standard of customer service for patient/client needs in all areas of service while upholding privacy and confidentiality at all times.
Successful applicants must have a current National Police Clearance (that can be validated within 3 months).

We can offer you
Hourly rate: $38.71 to $39.81 (inclusive of 25% loading) plus 12% Superannuation
