SUMMARY
Assists the Social Services Supervisor in coordinating and collaborating with a variety of external human resource services and internal resources departments or division to ensure the highest level of wellness and dignity for ADT/Residential Services program participants or residents. Recognizes each resident or program participant as an individual with unique and diversified conjunction with the Social Services Supervisor serves as advisor to the Quality Assurance Specialist and Director of Program Operations (DPO) in all matters related to an all-inclusive social services plan for the Key Training Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Assists the Social Services Supervisor with admission activities for all applicants including family interviews completion of the admission application security collateral information arranging for necessary evaluations and scheduling admissions staffing through the Client Review Committee.
- Assists the Social Services Supervisor with case finding and referral services for new and current program participants and maintains a log of referrals.
- Participates in inter disciplinary team and support plan meetings as needed.
- Complete case management activities including counseling regular progress recording and coordination with individual instructors.
- Assists the Social Services Supervisor with monitoring and reporting activities including quarterly progress reports and annual reviews to families support coordinators and others as specified in the habilitation planning process.
- Liaison with other service providers.
- Maintains records in a form which is confidential and meets the highest standards of clinical record organization including periodic review of records for content and organization coordination of sign-out procedures for records.
- Assists in coordinating transportation including scheduling drop off and pick up times liaison with families on transportation issues; liaison with drivers on an as needed basis.
- Assists in coordinating transportation including scheduling drop off and pick up times liaison with families on transportation issues; liaison with drivers on an as needed basis.
- Maintains liaison with families as related to a variety of needs including family counseling and home visits as needed; communication with families regarding accidents injuries and unusual incidents occurring at the Key Training Center.
- Assists the Social Services Supervisor with discharge planning and coordination for residents and program participants including referrals to other service agencies; coordination with APD; follow along activities after discharge; and recommendation for presentation to the Review Committee.
- Maintains liaison with APD and WSC workers as outlined in the Med Waiver Handbook and discharging social management for all residents and program participants.
- Serves as an advocate on behalf of all residents and program participants to ensure that their rights are respected in the manner in which CCARC Incs services are publicized and respected by the community at large.
- Performs other related tasks as assigned or required.

QUALIFICATIONS To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required. EDUCATION and/or EXPERIENCE
Prefer Bachelors Degree from an accredited college or university with major course work in social work and behavioral science.
At least one years experience in social work with a health and welfare agency is required; prior experiences with developmental disabilities preferred.
CERTIFICATES LICENSES REGISTRATIONS
Must participate in in-service education professional meetings to stay current in the social service areas necessary for this position.
OTHER SKILLS AND ABILITIES
Ability to deal courteously tactfully and efficiently with residents families program participants staff members and community agencies.
Emotional stability and maturity with a genuine interest in working with clients and program participants.
Capable of working a flexible hour/day or weekend schedule if needed.
Strong organizational and time management competencies.
Willingness to comply with in-service training or other mandates.
Must be able to recognize each resident and program participants individual unique and diversified needs through personal contact with the individuals as well as through interviews with family members.
Knowledge of AHCA APD and WSC service manuals for the State of Florida.
