Job Summary:

: Provides advanced psychosocial assessment of and brief targeted intervention for social emotional and environmental issues that impact client/family ability to optimally benefit from care. Implements family and individual psychotherapeutic interventions including supportive counseling and crisis intervention. Assists with development of discharge and disposition plans. Actively collaborates with team to meet client care outcome management and system improvement goals. Represents the VKC department/TRIAD on institutional committees/task forces. Provides leadership for department committees/workgroups. Supports TRIAD staff in meeting administrative and clinical standards and/or requirements.

KEY FUNCTIONS AND RESPONSIBILITIES:

Provides initial assessment and develops a plan of care according to The Joint Commission and CMS Best Practice Guidelines within scope of license as defined by the Tennessee Health Related Boards.
Completes psychosocial assessments and assists the team with development of the plan of care.
Provides education and/or consultation to clients families the treatment team and community partners.
Delivers targeted evidencebased interventions and integrates them into clinical practice.
Facilitates transitions to appropriate levels of care through inperson telephone or telehealth contact.
Acts as a clinical liaison among the patient family treatment team and community resources.
Participates and collaborates in system improvement initiatives.
Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards.
Contributes to organizational goals by participating in process improvement efforts and shared decisionmaking.
Provides leadership and supervision within scope of license to meet departmental needs.
Maintains confidentiality and privilege in accordance with Tennessee state codes and statutes.
Upholds ethical standards consistent with relevant professional association codes of ethics.

TECHNICAL CAPABILITIES:
Risk Management (Intermediate)
Demonstrates full command of riskmanagement practices for complex situations. Conducts analysis identification planning and control of riskmanagement procedures at the functional unit level. Performs prevention and contingency planning creating alternatives for common operational challenges. Applies common riskidentification methods (objectivesbased scenariobased taxonomybased and checklists).
Clinical Counseling (Intermediate)
Demonstrates mastery of clinical counseling in complex applications. Independently delivers highquality results without requiring support or instruction. Provides instruction leadership and examplebased coaching to others.
Crisis Intervention (Intermediate)
Demonstrates mastery of crisisintervention techniques in complex situations. Independently delivers effective intervention without supervision. Trains and guides others through modeling instruction and leadership.
Patient Evaluation (Intermediate)
Demonstrates mastery of patient evaluations in complex clinical applications. Applies a strong understanding of internalcontrol concepts within diagnosis and evaluation. Possesses sufficient knowledge training and experience to successfully deliver patient evaluations without requiring support or instruction from others.
Patient Assessment (Intermediate)
Demonstrates mastery of patient assessments in challenging practical applications. Possesses sufficient knowledge training and experience to deliver results independently. Able to train and educate by setting the example providing instruction offering leadership and elevating others performance while on the job.
