Collaborate to provide management and delivery of patient care plans
Support patients in Lewisburg and surrounding areas
Meet critical deadlines for charting
Self-manage time and schedule
Drive to/from patient locations

Schedule and Shift Details
Part-time Sat & Sun 8am-8pm

Travel
Local travel within the specified territory

Qualifications

Required

Associate degree or higher
Hospice (2 years)
1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
Valid Licensed Practical Nurse (LPN) in Pennsylvania (PA)
Do you have a valid Drivers License Reliable Mode of Transportation and Auto Insurance (all 3)
Are you okay with the required travel for this position
