Job Summary
Summary: Perform patient evaluations and plan implement and adjust individualized treatment plans. Work collaboratively with other healthcare professionals to ensure patients reach maximum level of function. Reports to: Rehab Manager Titles Supervised: NA Essential Duties & Responsibilities Provide direct patient care evaluate and reassess functional needs and consult with other specialist as needed and adjust care plan as indicated. Conduct therapy practice within defined standards of care as set by Illinois Physical Therapy Practice Act and department procedure. Delegate and supervise specific treatment to PTA and support staff in order to achieve patient s functional goals. Document evaluations treatment goals/plans and regularly update documentation. Maintain all required prescriptions and signatures. Minimum Qualifications: Education: Graduate of accredited Physical Therapist program and possess Illinois license (or temporary work permit) with prior demonstrable experience preferred. Experience: 1 year preferred in hospital setting outpatient experience
Shift: 8:00 - 4:30 Monday through Friday
