Job Summary

The Home Health Aide works under the direction of the registered nurse performing services for the customer which may include adolescent adult or geriatric as necessary to maintain the personal comfort and safety of the client in the addition the home health aide communicates and documents customer care outcomes.

Provides for the physical needs of the customer through observation and cognitive abilities.

Assists with basic personal cares such as feeding dressing weighing skin care nail care elimination needs transfers positioning exercise ambulation tube feedings (this is not an all inclusive listing)

Provides for nutritional needs by preparing food and fluids.

Performs light housekeeping duties that are incidental to customer care.

Observes and reports changes in the customers condition to the Home Health Aide supervisor or case manager/clinician.

Provides a safe environment for the customer.

Uses clean technique for cares.

Performs appropriate safety procedures when transferring the customer.

Provides for the psychosocial need of the customer.

Provides appropriate care based on customers age mental and physical status and Home Health Aide plan of care.

Accepts other duties as assigned to promote the accomplishment of organizational goals.

Requirement description :

2 Year of Experience home health hospice experience - Required
Home Health Aide Certification - Required Document
BLS (AHA or ARC) - Required Document
Driving license with a good driving record - Required Document
Adult Patient Population Experience - Required
Epic experience - Required

CERTIFICATION REQUIREMENTS :

ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : Weekends by Department Rotation
On Call Requirements : Every 5th Saturday
